• Application Form including two 2 x 2 ID pictures
• Basic Education Admission Test result
• Original Form 138 (Grade School Report Card)
• Original Certificate of Good Moral Character
• Original and Photocopy of PSA Authenticated Birth Certificate
• Data Privacy
• Recommendation Form
